Patient Portal is a secure online link that allows patients access to their electronic health records. Below is a link to the website where patients can either create a new account or log on if they have an existing account.
For patients without login information, click the Patient Portal box below, and then click on the registration tab in the top right corner. The screen will then prompt you to fill in your demographics as well as create a unique username and password (A unique password must consist of five characters long, one uppercase, one lowercase, and one number). An email address must also be unique to the patient (unless the patient is a minor.) At the bottom of the screen, register with the doctor provider that you saw for your most recent exam. When finished filling in the demographics section, click “Register” at the bottom of the page to continue on to the next screen. The portal will then prompt you that the registration process has been completed and a confirmation email has been sent to you. Allow our office 1-2 days to confirm your registration. You will then receive an email confirmation allowing you to complete your registration and then be able to see your most recent electronic health records online.
